Making a Copyright Claim
To file a copyright claim, please send a notification to dmca@utahformspdf.com. This notification should contain detailed information to help us swiftly identify and address the alleged infringement. Necessary information includes:
- Your contact information, including name, address, phone number, and email address.
- A precise description of the copyrighted work that you claim has been infringed.
- The specific URL or location on our site where the alleged infringing material is located.
- A statement declaring your belief that the use of the content in question is not authorized by the copyright owner, its agent, or the law.
- A statement made under penalty of perjury that the information provided in the notification is accurate, and you are authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
- Your physical or electronic signature.
Our Response to Copyright Claims
Upon receiving a copyright claim, Utah PDF Forms will promptly undertake an investigation into the matter. Actions taken may include removing or disabling access to the material claimed to be infringing. We are dedicated to complying with the law while also ensuring that disputes are resolved fairly and efficiently. The person who provided the content in question will be notified and given the opportunity to respond, which may include submitting a counter-notification if they believe the content was removed in error.
Counter-Notification Procedure
If content was removed due to a copyright claim, but you believe this was an error, you may submit a counter-notification. This should be sent to dmca@utahformspdf.com and must include:
- Your contact information, including your name, address, phone number, and email address.
- Identification of the material that has been removed or to which access has been disabled, and the location at which the material appeared before it was removed or access to it was disabled.
- A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification of the material to be removed or disabled.
- Your physical or electronic signature.
Following receipt of a counter-notification, Utah PDF Forms will provide the complainant with a copy of it and inform them that the removed content may be replaced or access to it restored in 10 business days unless the complainant files an action seeking a court order against the content provider.
